The Warren County Board of Developmental Disabilities Early Intervention team is highly committed to supporting your child and family with evidence- based practices, interventions and services. As the main caregiver for your child, you have the greatest and most important role in supporting your child’s development. Our role is to assist you along the way.
Primary Service Provider Model
担当のプライマリーサービスプロバイダーは、ウォーレン郡発達障害者委員会との主な連絡窓口となります。この担当者は、あなた、サービスコーディネーター、そして早期介入チームの中核メンバーと緊密に連携し、質の高いサービスを提供します。
健康と安全
お客様とそのご家族、そして当社のチームメンバーとそのご家族の健康と安全を維持するサービスとサポートを提供することが、当社の最優先事項であることをご理解ください。サービス提供は自然環境下で行われるため、対面でのサービス実施前に、ご自宅にいる方全員とサービス提供者全員の健康状態を確認することが非常に重要です。
遠隔医療と早期介入
Due to the nature of Early Intervention services, coaching and visits can occur in various places and in different ways. Early Intervention focuses on empowering parents to learn and implement strategies within their daily routines. 家族支援サービスプログラム
While enrolled with The Warren County Board of Developmental Disabilities, you will be provided information regarding the Family Support Services Program. This program is intended to promote family unity by financially assisting and/or find resources to meet the needs of the enrolled individual and supporting the family. This funding source is considered a payer of last resort for items that are therapeutic in nature, therapy co-pays, respite, and more. For more information, check out the link below!
Early Childhood and Agency Events
In addition to your Early Intervention services, periodically we will send out notifications of upcoming events within the Early Childhood division as well as The Warren County Board of Developmental Disabilities agency events and community/educational opportunities. Your email address will be automatically added to the list to receive information about Early Childhood specific events. Privacy and Parent Rights in Early Intervention
担当の医療従事者は、お子様の診察記録をすべて作成し、記録としてお渡しします。また、お子様の情報や関連書類は電子的に保管しています。記録には、保護者の方とのやり取り、個別家族支援計画（IFSP）、サービスやサポート内容、その他保護者の方からご提供いただいた書類などが含まれます。
早期介入からの移行
早期介入サービスは、お子様が3歳になった時点で終了します。ただし、サービスが不要になったとご判断された場合、またはウォーレン郡外へ転居された場合は、それ以前に終了となります。お子様が3歳になる頃には、担当チームが移行プロセスをサポートいたします。このプロセスは、ご家族によって異なる場合があります。早期介入サービス終了後のことを考え始めるにあたって、以下の資料をご参考になさってください。
